What is en bloc capsulectomy?
Some patients want to have their implants removed. En bloc capsulectomy is a surgical procedure used to remove breast implants along with the surrounding scar tissue, known as the capsule, in one piece. This method is particularly effective for patients experiencing complications such as capsular contracture, implant rupture, or concerns about potential implant-related illnesses.
Reasons for Implant Removal
Breast implant removal is a significant decision for many people with breasts. There are various reasons why you might consider this procedure. Common motivations include complications like capsular contracture, implant rupture, or changes in breast appearance over time. Some women choose implant removal for personal reasons, such as a shift in aesthetic preferences or lifestyle changes. At District Plastic Surgery in Washington, DC, we specialize in standard implant removal and en bloc capsulectomy, ensuring optimal outcomes and patient satisfaction.
Ideal Candidates
Ideal candidates for implant removal are those experiencing complications like implant rupture, capsular contracture, or those who no longer desire their implants. Suitable candidates should be in overall good health, non-smokers, and have realistic expectations about the procedure’s outcomes. Whether your reasons are medical or personal, Dr. Ramineni will provide expert guidance to help you make an informed decision about your implant removal surgery.
Consultation Process
Dr. Ramineni will discuss your medical history, current concerns, and goals during your initial consultation. A thorough examination will determine the best approach for your surgery, whether it’s a simple implant removal or en bloc capsulectomy. This approach offers personalized care, focusing on safety and optimal results.
How is the procedure performed?
Under general anesthesia, the capsule is separated from the breast tissue. The back wall may adhere fairly to the ribs and chest wall muscles. Ideally, the implant and capsule are removed as a unit. In some cases, fixing the chest wall may require removing part of the capsule in more than one piece. A drain is placed, the incisions are then closed and dressed, and a surgical bra is placed. In many cases, a patient may desire either placement of a new smooth round implant or augmentation with their fat. Commonly, a lift is performed at the same time if no implant is placed due to the excess remaining skin.
Recovery
This varies depending on if there are other combined procedures. Generally, most patients return to work in 1-2 weeks and return to the gym in 3 weeks.
Risks
- Poor cosmesis: Due to the removal of the implant, if no further surgery is performed, loose skin or deflation is likely
- Fluid buildup: A drain will be placed to remove serous fluid/blood, but in some patients, when the drain is removed, fluid may build up and may need to be removed in the office
- Pain: Due to the capsules’ adherence to the ribs, this procedure can sometimes be painful
- Asymmetry: If implants are removed and you have preexisting asymmetry, you may have asymmetry after the surgery without reconstruction
- Risks of associated procedures: If you have a breast lift, placement of a new implant, or placement of fat, the risks of those procedures would be relevant to this surgery
- Need for revision: Some patients may opt just to have their implants removed and then, in the future, decide that they desire reconstruction.
